Use this form to withdraw from your CiviqPass purchase. You can withdraw within 14 days of paying, without giving a reason. If you do, we refund the full price to the payment method you used and your access link stops working.

We keep your name, your address, the pack, the time of this statement and the outcome for 12 months, to show that you withdrew and when. Privacy
